Voters in North Carolina and Indiana were ready and waiting as polling places opened this morning. The nomination battle between Democrats Barack Obama and Hillary Clinton is expected to draw heavy turnout through the day.

The 67-page document lists conditions ranging from Alzheimers to diabetes as reasons to turn patients away during a medical crisis.
